I have never un-checked that option, so a title page always prints. I do not
have the problem you are describing.

You have to use the option to print the title page. I tried a number of
reports with that option checked. One of them worked fine. Others I tried
had a superimposed image of the title page on the first page of the report
rather than a separate title page.
