#1. All that I have on her is an e-mail address. Is there an
alternative template that would show her e-mail address? Maybe her
phone number?
If you only have an e-mail address and telephone number you won't be
wanting to use a mailing address, etc so why not put the e-mail address
and 'phone number instead.
#2. Further, I am confused by the Option at the bottom of the template
where it shows "Exclude from Reports." I would like for her name to
appear on any report, but not her address, e-mail, etc. At the same
time on the template's Output Preview, it shows her name with [ADDRESS
FOR PRIVATE USE]. Does this mean if I do *NOT* put a checkmark next to
"Exclude from Reports," only her name will appear?
Yes, that's what Ouput Preview means - it's a preview of what will show
when you create report.
If you check "Exclude from Reports" that is exactly what will happen -
that Source will not appear on Reports at all.
