According to "Getting It Right" by Mary H. Slawson (page 145), the
location should be entered thusly:

name of the body of water, followed by name of the jurisdictions....

Rio Guadalquivir, Posadas, Andalucia, Espana

Lake Washington, near Medina,King, Washington, USA

Certainly some rivers are quite long and you'd want to enter the
nearest town or other jurisdictional area.
