I do not use the Geo database at all, so cannot really help, I do know, however that there is now a unified one instead of the previous regional DBs. I have always found it pretty useless outside the US. I suspect that you are not quite correct in saying that there are no counties in England, they are. of course, a subset of states/provinces.
