I believe the best practice is to identify the place using the name that was used AT THE TIME. But then to insert a 'note' somewhere that gives the modern name … and perhaps explains all the names the place has had over the years (if you can do this … ). You could even note the latitude and longitude in this note, if you are so inclined.
BTW, I highly recommend the Windows software called Animap (www.goldbug.com) for county boundary changes within the United States. Its terrific software. I have no commercial connection with the software but I'm a very satisfied user.
