It is simply a matter of adding the child as a child of your sister, and then adding an additional set of parents to the child for the child's adoptive parents.
Assuming the child is a boy, select your sister and choose Add son. I would use the child's adoptive name for his surname.
Then, add the child's adoptive parents to your database in the usual way.
Then, to link them, simply select Add Son (link to existing person), choose the adopted child, and say "Ok" to the warning that will come up saying that the child is already a member of another family. The child will then be linked to the adoptive parents. To mark the relationship as adopted, with the child as the main person in family view, click on the Parents Icon (which should show [2]), highlight the adopted parents, and choose Adopted from the dropdown list on the Relationship to Father and Relationship to Mother fields.
You may also want to add an Adoption event to the child for reporting purposes.
