In the Name Tag Window there is a radio button on the right side giving
you the option of outputing to a file.  This will create a text file in
whatever directory you choose with any name you desire.  This file can
easily be edited to give you what you want.

If you use the "Address: Line 2" field to enter the date added in the
individuals address entry, you can choose to have that printed on your
