Comes in original Apple box with instructions and charge cable. Also will include a beat up but perfectly functional Otterbox Defender (black and white). The phone itself looks perfect to me, glass and back, because it spent it's life so far in the Otterbox. It also works perfect and holds a great charge. I think you could get it to work on AT&T with a new sim, but I really don't know for sure. It is clean, and can be activated on T-Mobile. I would keep it, but I switched carriers.
