I think it's working as intended - not sure what the docs. say but my 
interpretation is it's a way of saying to Leo, "*my* first lines *must* be 
first, don't go sticking any of your lines in front of them".  So you still 
have to put your first lines first, it just makes sure Leo doesn't put sentinel 
lines in front of them.
Also, I think the need for it goes away with @clean, whatever's first is first, 
you'd only need it with @file.
Finally, as another subtlety, remember things like @language don't get written 
to the external file with @clean, so they're not going to be before anything.
