I thought we dropped module-init-tools in favor of kmod.  However there 
are several places that still mention module-init-tools: 
dependencies.xml, whatsnew.xml (commented out),  patches.xml, 
packages.xml, chapter06.xml (commented out), packages.ent, and patches.ent.


Matt, do you want me to clean these up?
