Create it with the commands described in the book. On the page about compiling the kernel
loadkeys -m /usr/share/kbd/keymaps/[path to keymap] > \
drivers/char/defkeymap.c
Obviously, change [path to keymap] for the map you want. the one that works for me is /usr/share/kbd/keymaps/i386/qwerty/uk.map.gz
You'll need to choose the one that works for you.
