extradata is still my primary suspect, I can't be sure that you filled it with the correct data because I have no access to the source stream. Comparing it to whatever is filled into that field by rtpdec_mpeg4.c would be very helpful, but again, then you need to start doing test with rtpdec_mpeg4.c.
