The very scenario you describe occured in Texas where a criminal thought he 
could take down a waitress.

The owner saw what was happening and drew a shotgun on the perp.  Of the five 
patrons, two were carrying and drew their weapon the perp.  The perp very 
slowly put his revolver on the counter and was arrested soon thereafter.

The issue isn't that you can imagine a guerrila scenario where a perp has the 
advantage.  What IS important is that the situations in which the perp will be 
advantaged become vanishingly small as more and more people carry.  

The Reality Factor is what is important here -- what really happens when people 
carry is that they are not victimized and prevent others from being victimized. 
 At a certain point, the smart criminals don't play Texan Roulette and the dumb 
ones end up dead.  After a fair number get lead poisonings, attempting to hold 
people up except under the most advantageous conditions, is just a losing -- 
and deadly -- proposition.

  I don't get it. Seriously. Self-Defense? Guy walks up behind you at
  the WalMart checkout line. His intention is to shoot you in the back
  of the head and take your wallet. This is how he makes his living
  every day. So now he's pulling out his gun, and quick as a wink he
  positions it an inch away from the back of your head. Explain your
  Self-Defense strategy.
