I generally use las2txt to create the text files ( x,y,z,intrensity,number of returns, return number) and then aggregate all of the text files to a single file and use r.in.xyz in grass. With r.in.xyz, you can designate which colums are x, y, znd z. For intensities, I use the x,y values and use the intensity value as the z value, preforming a mean statistic on the intensity values in each cell . I have done this with text files up to 140 GB in size. Recently, I have been filtering by return number to get the mean intensities of the last returns to try to help determine moist soil/wetland footprints.

It would be similar to creating a DEM. Theres an example here http://www.locuscor.net/wordpress/ of piping the output of las2txt. You'll need to switch references around to refer grass to the intensity value.
