I chime in just to say it also happens to me, but I have not
investigated much further than you nor I can offer real help at this
moment. Same setup, same issue, except that whenever I plug an external
monitor I see a third, very low resolution display reported in GNOME
Settings; and disabling this pseudo-display makes the slowness
disappear.
I also noticed that the gnome-shell process is what takes all the CPU.
I can't remember if I ever plugged an external monitor on this laptop
before flashing Libreboot. Maybe we should begin by ruling out the BIOS
using LenovoBIOS, testing a different desktop environment.

resolution to 1024x768 (with mirror display) on both displays seems

Maybe the GM4500HD in the newer thinkpads can handle this better.
As a side note, probably not possible, but has anyone every
researched using digital video ports from thinkpad docks without the
dock. It seems like a rather heavy accessory just to enable
DVI/Display port on X60/T60/X200/T400. If only we could some how
leverage the bottom connector on these thinkpads for a single display
output. I suppose T500 might be the only way to avoid carrying around
a docking station.
