Description:
LO Writer crashes when I attempt to insert an Index or TOC in my document.  I
don't normally use this feature; but, I'm trying to learn about handling large
document by following this online tutorial
(https://oercommons.org/courseware/lesson/22044/student/?section=17).


Steps to Reproduce:
1. Open a new document in Writer
2. Click on Insert > Table of Contents and Index > Table of Contents, Index or
Bibliography

Actual Results:
The TOC, Index and Bibliography dialog flashes briefly and Writer closes.


Expected Results:
The TOC, Index and Bibliography dialog appears allowing user to create and
configure a TOC, Index, or bibliography.


Reproducible: Always


User Profile Reset: No

Additional Info:
The document I was using can be restored upon restarting Writer.
