Description:
The table of contents was supposed to show only headings with the style of
heading 1.
However it included all titles including those styles as heading 2 and heading
3.
Editing the TOC made no change.
Deletng the TOC and re-creating it anew did not fi it and gave the same result.

Expect to see a TOC that had only the titles of chapters. These were formatted
as "heading 1"
