Description:
Ordered lists that start at a value other than 1 lose their starting number
during import.
Steps to Reproduce:
1) Save as markdown file (input.md) with notepad.
2. Second item
3. Third item
4. Fourth item
2) Open LibreOffice
Actual Results:
The list is imported starting at 1.
Expected Results:
The imported list should preserve its starting value of 2.
Reproducible: Always

## Impact
The list numbering defined in the Markdown source is altered.
This changes the meaning of documents that intentionally use a custom starting
value.
