Description:
having a document (like a master doc) including a doc2 (which defined an
alphabetical index based on a concordance file) does not index the main
document. manual index entries work fine.

Steps to Reproduce:
when using the attached documents, all paths need to be re-done.
1. create doc1 with text.
2. create doc2 defining an alphabetical index and base it on a concordance
list.
3. include doc2 in doc1 (eg as a section), update index.

Actual Results:
nothing is shown

Expected Results:
the words in the concordence file should be indexed and shown in the index.

Reproducible: Always
