This was a Writer form document.
I have a current version of the document but did not save the one associated
with the crash.
I had been having an issue with the form extending onto a second page and had
removed the logo from the header.
I was attempting to add a larger version of the logo as a watermark to the
portion of the document behind the form fields and the image was not clear
enough to be of value so I deleted it.
I believe I was then or when add/editing a form field when LO crashed.
I have not attached the current version of the document file as the club logo
is not available for public documents.
