For me it opens in LibreOffice 4.1 and 4.0.4 on Windows XP, it just takes a
long time. After i opened closed the dialog a few times it still takes around
10 seconds to open the dialog, first time it's even worse. But I have the same
issue with the menu command that also takes a lot of time to load.
If you use File -> New -> Templates, are you able to show the dialog then?
