They were very simple forms, standard fields, nothing weird or tricky done with them. I have long since uninstalled all versions of 4.2.2 - but did you simply try creating a new template with the same fields? Maybe I can find the time later today to create a new form/template with the same fields in 4.1.5, then try opening that on a PC with 4.2.2 installed and see if the problem persists. Then I'll try creating the same template with 4.2.2, and see if still persists...
