I am doubleclicking these random "Heading" entries in the Navigator window.
Each time I click doubleclick them it takes me to a different page, and
sometimes updates the page number in the navigator window.   These headings
also appear on PDF exports of the file.  There is no way to delete these
entries.  They do not belong to (or appear in) any index or in any special
style.

Steps to reproduce:
1. Write a large document
2. add in headings and what-not
3. open navigator window and enjoy the show

Current behavior:  not working logically

Expected behavior: to not have random blank entries or the magical ones that
automatically change their links and such.
