Could confirm this buggy behavior for OpenSUSE 12.3 64bit rpm Linux and LO
4.3.0.0beta2
It is not only a problem of a template. I have the same problem with an
existing document when I have created my own styles for the headers. When I
reopen the document and will refresh the index all content of the index seems
to be gone. The properties have changed from "Outline" and "Index marks" to
"Additional Styles".
