Under GNU/Linux using v4.3.0.3 Build ID:
08ebe52789a201dd7d38ef653ef7a48925e7f9f7 the problem as described still exists
for the DOC file format (works as expected for DOCX file format). Workaround:
Use Tools > Outline Numbering menu to assign headings to outline levels for DOC
file format.

Under GNU/Linux using v4.4.0.0.alpha0+ Build ID:
4aa9b041de3129f19b48e66d349f48657b73f33e (2014-07-19) it appears to be solved
for DOC file format (and also works as expected for DOCX file format). When
opening the provided DOC the Heading 1 paragraph style is already assigned to
outline Level 1. 

If someone else can confirm this behaviour this report may be able to be
RESOLVED as WORKSFORME (unless someone can point to a specific fix, in which
case it can be FIXED).
