Reproducible. There is indeed no header in the docx (checked with Word for Mac
2011). I'm not sure about your 'second page' (I can't see one in your attached
ODT file either).
Tested using Mac OSX 10.9 with LibreOffice Version: 4.4.0.0.alpha0+
