However, you should be able to reproduce it with a simple ODB database consisting of a single table named contacts, whith a field for firstname and a second one for lastname. In the standalone form you then have to select that database and its contact table as data source LibreOffice gives you the possibility to "register an address book". This feature creates a read-only address database which allows you to access values of your external address book. On the Mac I use the OSX Contacts.
