Yes ideally both problems should be looked into when a dev looks into this.

Looking in the odt files in steps 3 and 6, after step 3 you have these files
missing after step 3

/Object 1/content.xml
/Object 1/meta.xml
/Object 1/styles.xml
/ObjectReplacements/Object 1

These file must relate to the chart which has gone missing. Priority increased.
