Steps:
in a Calc spreadsheet, I insert a picture
it shows fine
I save it
I reopen it
Now the picture is gone and there is only an outline showing where it was
What should happen:
By default, the insert should be embedded in the doc.
Reason: if I give it to someone else, and they do not have internet connection
/ the url is not active / they do not have the pic on their disk, it will not
show

The example I have attached is Calc, but the same thing happens in Writer.
I now added another sheet and again did an insert.
I went to navigator / images / and changed drag mode to insert as copy.
saved. re-opened and see that drag mode has reverted to "insert as link".
Yet, when I insert it the file dialog has a box which you tick to add as
hyperlink - this is unticked (see the picture in sheet3)
