Using the file name as a text alternative is a bad practice. Please consult the
W3C's Authoring Tool Accessibility Guidelines 2.0:
https://www.w3.org/TR/ATAG20/#gl_b23 (unfortunately, the document was never
finalised).
The proper way of getting text alternatives is prompting for them. See feature
request 39558: "ACCESSIBILITY: always prompt for text alternative when
inserting picture to create accessible documents by default" at
