It does not make a difference whether I save the (new) document before adding the header or not. Since this seems to require more testing on other platforms, I am not changing the status to "WorksForMe" for now. One final note: If the issue still exists on other platforms, I think it should be fixed by preventing form controls from being added to a document's header (or footer). The reason is that such form controls are simply not perceivable to screen reader users.
