I'd think drilling holes would be the worst thing you can do - metal conducts heat better than static air and given you're not going to be drilling holes in the keyboard itself (or creating any other air passages), you're not going to get any more airflow through there by drilling holes in that sheet of metal. You'd be better off perhaps putting some heat transfer compound between the heatsink and the "heat plate" (the thing with the "hot surface" warning sticker and the thing I'm presuming you're considering drilling) and between that and the keyboard if you want to transfer more heat through the keyboard.
As for PCMCIA fans, you can buy them, in Australia I've seen Jaycar Electronics carry them ... basically a PCMCIA card with perhaps a 5cm square thing poking out the side and a fan in it. The top of the PCMCIA card has a few slots ... basically it forces air into the top of the PCMCIA socket of the laptop (ideal for the Libretto given that's where the CPU is). www.dansadata.com/jfans2.htm has a picture of the one I refer to.

John... I had the same problem when I O/Ced my 100. No matter how I tried to tweak the setup with things like reapplying heatsink grease, moving my hot Xircom dialup/ethernet combo card down to the EPR et al, I just couldn't keep the system from experiencing all kinds of overheating problems. Every CPU is totally different in their specs from the next one coming right out of production, so some O/C well, and others not.
That said, seems a lot of people who experiences overheating problems with their 100 MBs at 266 have had success clocking back to 233... something I'm considering since my 110 MB died... but I'm not good at micro soldering/sugery.
On the topic of cooling the system down... I recall someone somewhere along the way pointing out that a number of years back there used to be PC card fans made specifically to cool down notebooks. I never explored that avenue of possibility very far for some reason. Maybe I just couldn't find any.
