You'll need a separate engraver anyway, because the name not only
has to be in the margin,
but also in a separate text over the first note of the new instrument.
I was thinking that I would define a variable or function to add the
markup above (e.g., in a box) and simultaneously \set Staff.instr --
are you thinking it would/should be handled by a single instr-change
call?
