lilypond-book uses a custom paper block, and this upsets the default
formatting of this example.  Perhaps I should add a note about this?

No. This is one occasion where changing the layout block is
justified.  By default the indent is set to the paper-width divided by
14, which for A4 gives an indent of 1.5\cm.  To show the default
behaviour use
@lilypond[verbatim,quote,indent=1.5\cm,relative=2]
for this example.

However, this is still not quite enough to centre "Flute" due to the
length of the second instrument name.  Perhaps both instrument
names should be shorter to illustrate the default behaviour properly.
