Why is the vertical extent of the strut ignored?
Because side positioning is primarily based on (vertical) skylines,
not extents.
Thanks. What is the reasoning behind this? For me, this behaviour is
unexpected.

If side positioning were done with extents only (like it used to
be done in the past, I believe), it would be less accurate. For
example:

\version "2.23.81"

{
  \override Slur.show-vertical-skylines = ##t
  % Toggle \override to see the effect
  %\override Slur.vertical-skylines =
  %  #(ly:make-unpure-pure-container ly:grob::simple-vertical-skylines-from-extents
  % ly:grob::pure-simple-vertical-skylines-from-extents)
  c'''2\tweak details.edge-attraction-factor 10 (^"a" g''' c''')
}
