I'm afraid I'm rather naive and don't know how to do this -
libguile.so.12 is in /usr/local/lib
guile is in /usr/local/bin

Do I need to set an environmental variable?

Yes, you need guile in your load path.  For midi.so that's a bug, sort
of, but you'll need that for lilypond anyway.
