Albert,
    In your example you forgot the "\"character before "thumb". This would
result in your example either doing nothing or generating an error. The
"\thumb" instruction is for the left hand so you are giving two indications
for the left hand and none for the right. I believe what you need is
something like: <a-4>^\markup { \italic { p } }

This worked in v2.2.5 but I have not used this since upgrading to v2.4.2.
Try a small example and if it does not work, run the convert-ly script on it
and it should update the syntax if it is not correct for your version.
