For me, after putting a space after #'header:title, the problem appears in 2.18.2 and 2.19.83, but disappears in 2.21.0 (master on March 29 from http://partitura.org/index.php/lilypond/ <http://partitura.org/index.php/lilypond/>). Replacing set-global-staff-size calls with layout-set-staff-size in \layout blocks (see previous message) fixed it in 2.18.2 and 2.19.83. Jean Abou Samra
