The { do } block isn't part of the \alternative block, so
lilypond
interprets the volta as having a repeat count of two, with only
one
alternative.
Ok, thanks. That doesn't get me much closer to a solution that
engraves and plays correctly, then.

It's not possible to do this within a single \score
block. Have a look at Notation Reference 3.5.4 "Repeats
in MIDI".
