The opt out pages for My Health Record have at the bottom:

So I ran the pages trough the the aChecker accessibility test: https://achecker.ca/

The introductory pages were okay, but problems started at the first of the three pages where the user had to start inputting data:

This page had 3 "Known Problems" and 169 "Potential Problems".

The My Health Record opt out function does not appear to follow the Web Content Accessibility Guidelines (WCAG) 2.0 level AA. The requirement to do so was established in the Sydney 2000 Olympics case in the Human Rights Commission. I was one of the expert witnesses: https://www.w3.org/WAI/bcase/socog-case-study

ps: The Google Page Speed Tool rates the Page Speed as "Average" and Optimization as "Low" at 34 / 100.
