Everything looks good from the log. The OSA driver loads and the interface gets configured. Is the /28 subnet mask correct for your system? That equates to 255.255.255.240.
What system are you pinging from? Is it on the same subnet (Guest LAN?) If not, are there any firewalls in between that might be dropping icmp packets? You might just try re-IPLing from 200 and see if it goes away (without doing a complete reinstall just yet), because it might just be a glitch.
