Well, I always build the volume group from the multipath devices but
sometimes I still see duplicates when I pvscan...
What is the solution? Is there a way to exclude the /dev/sdx from lvm?

I've seen this issue before when the volume group is created from the
/dev/sdXX instead of the multipathed devices.
Running PVS should tell the tale.

Sadly, I was not there when the problem happened and none of the commands
were entered...
I can't recreate the problem as this is a production server and I don't even
know if it will happen again when I move the guest back to its original
z/VM.

pvdisplay

according to that output, are you missing a volume group or a pv ?
