Did you explicitly 'dd' off any labels from prior usage ?

Also remember that LVM looks beyond the first sector (and
offset multiples), and needs to be 'scratched' with the tools
in the lvs2 suite.  the LVM is sneaky enough to sniff for and
then to 'see' old layout data, and to try to assemble a
partially broken drive into its coverage
