I have had this a couple of times.  From past experience the only sure-fire
way to get things to work is to tell RMK that you have a reproducible error! 
In practice, I have only seen this since I upgraded to give me silly amounts
of memory (130Mb) while it is fairly cheap, so I suspect it might be related.
 I have also had the install sequence fail a couple of times, although not in
the way that you describe - it is usually the package installation that fouls
up on a couple of packages for me.  Having said that, I have also been able
to install and run with the new modules installed and my full complement of
memory working, so I am not entirely sure that I have nailed the problem down
(yet).
        However, it is probably worth running the installation with mem=16M
onto a clean disc and seeing if that helps.
