Starting to add in a user guide document (.tex) for btt - describing how to invoke it, and what the output means.

The first patch rearranges the output such that only the "interesting" stuff is put out by default, some more verbose (and less useful?) stuff is available with a new -A option. (Cleans up the output - especially for simple stuff.)

The second patch contains the prototype document stuff - in particular, it is missing some graphics I have in mind. [[Jens: I'll need to have some sort of graphics files in addition to the .tex file (.eps, .jpg, .png?) - what's the best way to get that done with git? Just treat them like text files (which they are) but they can be rather *big*...)]] I've got a Makefile and directory structure that mimics the blktrace directory, Jens - feel free to munge with as needed. This also bumps the btt version to 0.99.1 to make it align with the documentation (and the changes in the first patch).
