Its strange that you say this, because I have had nearly the same
identical problem with my own ISP- bellsouth.net. I have had my 3com
ImpactIQ external modem for nearly 18 months and have never had a problem until
the last few weeks. I went through my Impact AT commands and went
through all the values and made sure they were correct.   
I made a change in the chat-ppp0 and added
             +---I changed my default AT W0 to AT W2 (personal choice)
TIMEOUT 5 'CARRIER' instead of
'CONNECT'
My ISDN connection reliability is now back to 'normal' with only an
occasional misconnection.  I suspect a 'bad' software update has occurred
at the ISP and is being corrected when discovered or my TIMEOUT 5 value
'fixed' the problem. It may be something that only shows up with the
3comImpact.
