The patch titled
     ext-ea-block-reference-count-racing-fix-fix
has been removed from the -mm tree.  Its filename was
     ext-ea-block-reference-count-racing-fix-fix.patch

This patch was dropped because it was folded into 
ext-ea-block-reference-count-racing-fix.patch
