Quite a few possibilities out there...

1. You are not booting into rc level 3 or 5.
2. Your symlink is not correct.
3. /etc/rc.d/init.d/qmail is not executable.
4. /usr/sbin/sendmail exists.
5. qmail-start doesnt exist in /var/qmail/bin - forgot to do a make
install ???
6. Check whether /var/lock/subsys/qmail exists and of which date ...

Other than that there are a few mistakes which are not related to
startup of qmail.

1. Killing qmail-lspawn doesnt bring down the qmail system. Kill
qmail-send.
2. export PATH=/var/qmail/bin:$PATH
3. qmail-start '|preline procmail' splogger qmail & - the ampersand is
supposed to be in the same line.

Try adding echo statements to see what all is actually getting executed
before conking off.
