This, I believe, would require client side software. An extra
IE/Netscape plugin, maybe. But, with squid comes a pam_auth.c, and I
believe newer PAM versions can authenticate users from an NT
domain. Also, IE can store proxy password (so does konqueror ;-), so
the user has to enter his password only once.

I think PAM has facilities for process accounting (primitive, maybe)
as well.
