The most common cause is type of telephone line. Most dialers
are set to tone dial by default. Are you on a pulse dial type
of exchange ?

Are you enabling dial tone  detection with  X2 or  X4 in your
modem string ? Normally X3 (line busy) is enough, in case you
want X* in your init string. The default X0 is good enough.

Can't think of any other reason.
